3) Please complete below form and get it signed/ stamped a from an allopathic medical practitioner and bring it to the venue at the time of the event. Failing which, you may not be permitted to be participate to event.
4) I agree to sign an indemnity form at the venue taking full responsibility of any immediate or delayed injuries resulting from the MMA fights. Hear by, reliving the organizers totally from any responsibility arising out of the above.
5) Bring BC, HIV1 & HIV2 and Hepatitis B reports of past 30 days.
